First Office Visit

The length of the first office appointment is an hour to an hour and a half. At this visit we discuss your past and current medical health history, perform physical exam, and discuss treatment options. Below is a link to our patient intake forms that are to be filled out prior to the first visit. The forms may be printed from the link below, and filled out at home or you may come in to the office 30 minutes before your appointment and fill out paperwork provided by us.

Return Office Visit

Return office visits range from 30 minutes to an hour in length depending on how much information is to be discussed. Brenda, our office manager can help to establish the appropriate visit length when you call to schedule.

Meet and Greet Visit

A "meet and greet" is a free 15 minute visit for a new patient to meet a physician at our clinic. At this visit, patients can get to know the physician and learn more about what we offer at our clinic. During the "meet and greet", a diagnosis will not be made, nor will examination occur or treatment options discussed. Medical advice will not be given at this visit.

Dr. Summer Jo Nowicki

The third Friday of every month, Dr. Summer Jo Nowicki offers a menopause shift from 9:00am - 1:00pm


All the practitioners in our office are contracted with most major insurance carriers. However, individual plans vary, and it is the patientís responsibility to verify coverage before the first appointment. Insurance coverage can be verified by calling the 1-800 number located on the back of your insurance card. You will need to provide your ID number and the practitionerís name. Cash patients are required to pay for services at the time of the visit.
