New Patients

Meet and Greet: Our clinic offers a free 15 minute visit where new patients can get to know our doctors and learn more about what we offer at our clinic. During the meet and greet, no medical diagnosis or treatment options will be given.

First Office Visit: A first visit with a patient may last around one hour. Our doctors take the extra time to gather information, do an appropriate examination and teach you about managing your condition and improving your health.

Forms: Please download and complete the intake form prior to your visit or you may come in 30 minutes prior to your visit to complete the form in our office.
